The Toowoomba Hospital was established in approximately March 1863 with the appointment of a surgeon, the formation of a governing Committee and the gazettal of the rules and regulations governing its administration in August 1863.

Under the Rules and Regulations 1863 the hospital was "to afford relief to the sick and the surrounding district, and to supply, as far as possible, the want of a Benevolent Institution." In line with the growth of the population within the city and the surrounding areas, the hospital has continually expanded the range of hospital and medical services provided to meet current needs.

The Toowoomba Hospital Management Committee appointed in 1865 was replaced following the passing of the "Hospitals Act 1923" by the Toowoomba Hospital Board. Under the "Health Services Act 1991" the Board was abolished and the Darling Downs Regional Authority took over responsibility for the hospital. From 1996 Authorities were dissolved, and District managers and District Councils to monitor services were appointed. In 2007 the Toowoomba Hospital is, within the Southern Area, part of the Toowoomba and Darling Downs Health Service District.
